Why one height result is not a general rule

A 2021 review of pillow-height research identified body dimensions, head-neck contact pressure, and neck-and-shoulder muscle activity among the variables researchers have studied. It found insufficient evidence for an optimal height range and no firm conclusion on optimal height for people lying on their side or back.

The review did not assess the quality of its included studies. Most were short-term studies involving young, healthy adults, and two authors were industry employees. It does not establish an exact height, a firmness direction, a mattress rule, or a clinical or sleep benefit.

What one side-lying study actually found

A 2015 laboratory comparison involved 21 adults without symptoms. They lay on their side with 5 cm, 10 cm, and 14 cm foam pillows. Height was associated with measured neck and upper-back muscle activity and rated comfort in that setup. The 10 cm test pillow received the highest comfort rating within that comparison.

The study was a short laboratory test, not an overnight effectiveness study. It used one foam-pillow setup and three fixed heights. The 10 cm result is not a recommended shopping height, and the study does not establish pain relief, sleep quality, treatment, or the validity of a commercial fit process.

A broader systematic review of 35 pillow studies found that results differed by design and population. Pooled chronic-neck-pain studies did not show clear improvement in sleep quality. Findings about rubber or spring pillows cannot be transferred to YOUP or memory foam.
